Summary of Accident:
A factory worker was killed when he was struck by a container. During the incident, the container was being lifted by an overhead travelling to discharge its load into a furnace. While the container was being lifted, the victim was underneath to adjust its position. Suddenly, the hook holding the container came off and the container fell and struck him.
Contributing Factors :
The hook used to lift the container came off, causing the container to strike the victim who was underneath to adjust its position.
Results of Observation :
Lack of safe work procedures for the work carried out.
