Summary of Accident:
A plantation worker was killed when an earth moving machine struck him. During the incident, the victim was accompanying the machine which was crossing a wooden bridge. When they were at the middle, the bridge suddenly collapsed and the victim, who was beside the machine, fell together with the machine into the ditch. He was pinned down by the machine and died at the scene.

 
Contributing Factors :
The victim was accompanying the earth moving machine in a dangerous manner, i.e. walking beside it while it was moving and crossing the wooden bridge.

Lack of safe work procedures.
